The item upgrade (click through the following document) lets players boost an item's gear rating up to 700 points and add a bonus. Items can be upgraded by having them in inventory before heading to the kiln.

Certain items can be upp'd to increase their stats (like a perfectly made breakpoint armour that can be rolled to use in melee PvP) while others like weapons don't change much post upgrade, and therefore aren't worth upgrading.

Weapons

Weapons can be upgraded to improve their numeric stats like the chance to crit, attack damage as well as PvE and damage to PvP. Weapons have a flat-rate bonus that increases damage per second. Many weapons also have a fixed bonus which is activated when three or more identical weapons are used in a group. This bonus may be more than the damage that an upgrade alone can do.

Armor items can be upgraded to boost their stats. For instance, if you have chests or helms it will boost its base defense while gloves and belts improve their required strength and dexterity. Armor also offers the option of rerolling its Enhanced Defense percentage, which means that it could change its roll to a lower value.

The upgrade template of a weapon will show the new stats it will gain after upgrading along with the relative value of those stats. This means that a weapon will gain 3% attack damage if it is upgraded from an 8 attack damage sword to an 11 attack damage sword. However, the 3% doesn't mean that it will strike 3 times less often because it's an absolute statistic.

A weapon can also be rerolled by pressing the green button, which is accompanied by circling arrows in the item menu. This will roll an item and alter its stats, but it will not reset the weapon's levels, or tie it to the account status.

Armor

A few armor upgrades can make a huge difference to your survivability. Certain sets can offer MASSIVE increases in poison, curse, fire and magic resistance, making it easier to navigate through tough terrain. Generally speaking, the more defense your character has the more value you get from each point of health and it's crucial to improve your armor as much as possible.

You can upgrade your armor by visiting one of the Great Fairy Fountains. There are four of them and each one requires you to complete a special side quest involving the Stable Trotters musical troupe. After you've completed the main side quest and have completed the fountain in question you'll be able to contact any Great Fairy to open up the armor upgrade menu.

Each piece of armor can be upgraded up to three levels, and the amount of material and Rupees needed for each will vary. In general, you'll need at anything from three to five Red Chuchu Jellies and ten Rupees for the initial upgrade items, then anywhere between five Rubies and 10 Fire-Breath Lizalfos Horns for each subsequent upgrade. The Champion Leathers, for instance can be upgraded to an increase of 5 defense rating, which will allow you to stay ahead in frigid environments with a large number of foes. The Snowquill Headdress, on the other is able to be upgraded as high as +20 defense, which means you can survive a raid with your comrades.

You can't upgrade your armor in the same way as weapons. If you're playing as a Warrior it is best to focus on upgrading your armor to increase physical power or the power of spells. It's also a good idea to save your armor upgrades to use for armor you'll still be wearing at the end of the game, such as the Ring of Lightning and Spectral Shield, or the helmet you'll wear for the final boss fight, the Monter Parts Merchant's mask.
